Condición del hueso alveolar y espacio periodontal en terceros molares inferiores retenidos evaluadas en radiografías periapicales, Lima – 2024

Abstract

El presente estudio tuvo como objetivo determinar la condición del hueso alveolar y del espacio periodontal en terceros molares inferiores retenidos, evaluados mediante radiografías periapicales en Lima durante el año 2024. Se analizó una muestra de 132 radiografías seleccionadas mediante muestreo aleatorio simple, utilizando una ficha estructurada como instrumento. En términos generales, el hueso alveolar presentó una condición favorable, con integridad relativa conservada en más del 90% de las piezas analizadas, cortical interna continua en más del 80% y trabeculado regular en más del 89%. En la posición vertical, se observó una condición ósea estable, aunque con adelgazamiento frecuente del espacio periodontal en la región periapical. En la posición mesioangular, también se conservó la estructura ósea, pero se evidenciaron alteraciones del espacio periodontal, particularmente en las regiones mesial y distal. En la posición horizontal, la integridad ósea fue óptima, sin embargo, se identificaron mayores alteraciones periodontales, predominando el ensanchamiento mesial y el adelgazamiento en la región periapical. Los hallazgos permiten comprender cómo varía la condición del hueso alveolar y del espacio periodontal según la posición del tercer molar inferior retenido, siendo la posición horizontal la que presentó las mayores modificaciones en el periodonto. Estos resultados pueden ser útiles para la evaluación diagnóstica y la toma de decisiones clínicas en pacientes con terceros molares retenidos.

The present study aimed to determine the condition of the alveolar bone and the periodontal space in retained mandibular third molars, evaluated through periapical radiographs in Lima during 2024. A total of 132 radiographs were analyzed, selected through a simple random sampling method, using a structured data collection form. In general, the alveolar bone showed a favorable condition, with relative integrity preserved in more than 90% of the examined teeth, a continuous inner cortical bone in over 80%, and a regular trabecular pattern in more than 89%. In the vertical position, the bone condition remained stable, although frequent thinning of the periodontal space was observed in the periapical region. In the mesioangular position, bone integrity was also preserved, but alterations were identified in the mesial and distal areas of the periodontal space. In the horizontal position, the alveolar bone condition was optimal, yet more significant periodontal changes were found, particularly mesial widening and periapical thinning. These findings help to understand how the condition of the alveolar bone and periodontal space varies depending on the position of the retained mandibular third molar, with the horizontal position presenting the most notable changes in periodontal structure. This information may support diagnostic evaluation and clinical decision-making in patients with impacted third molars.
