Mortalidad y factores asociados a estenosis traqueal en pacientes COVID-19 con intubación prolongada del Hospital Carlos Lanfranco La Hoz, 2020-2021

Abstract

Objetivo: Determinar la mortalidad y los factores asociados a estenosis traqueal en pacientes COVID-19 con intubación prolongada del Hospital Carlos Lanfranco La Hoz durante el 2020 al 2021. Materiales y métodos: Estudio observacional, retrospectivo y de corte transversal. Se consignaron 252 pacientes atendidos en la Unidad de Cuidados Intensivos COVID (UCI COVID) del Hospital Carlos Lanfranco La Hoz durante el periodo 2020-2021 de los que 162 acataron los parámetros de inclusión. Se alcanzó una muestra de 100 pacientes. El instrumento fue una ficha de recopilación, se empleó el programa STATA versión 14 para el análisis de datos. Resultados: Se recopilo información respecto a 100 historias clínicas conforme al criterio de elegibilidad. Se evidencio un porcentaje de mortalidad del 78% en pacientes con estenosis traqueal postintubados atendidos en la UCI COVID del Hospital Carlos Lanfranco La Hoz en el periodo 2020-2021. De estos, el mayor porcentaje fue de sexo femenino con un 81,25%. Así también, se demostró un sumo porcentaje de mortalidad en pacientes entre los 70 a 90 años, representando el 93, 55%. Los factores cruciales relacionados a estenosis traqueal resultaron ser la intubación difícil y el grado 4 según Mallampati/Cormack con un OR de 56,83 y 88,00 en ese orden. Conclusiones: En nuestra región, la estenosis traqueal ha presentado una alta mortalidad (78%) que evidencia su gravedad en relación al contexto COVID - 19. Así también, siendo una complicación que afecta notablemente la calidad de vida de los supervivientes a este event o. Además, se reflejó una mayor preponderancia de mortalidad en el sexo femenino y el grupo etario adulto mayor. Los factores asociados a estenosis traqueal con mayor significancia fueron la intubación difícil y el grado 4 según Mallampati/Cormack.

Objective: To determine mortality and factors associated with tracheal stenosis in COVID-19 patients with prolonged intubation at the Carlos Lanfranco La Hoz Hospital during 2020 to 2021. There were 252 patients treated in the COVID Intensive Care Unit (COVID ICU) of the Carlos Lanfranco La Hoz Hospital during the period 2020-2021, among whom 162 met the inclusion criteria. A sample of 100 patients was acquired. The instrument was a collection sheet, using the STATA version 14 program for data analysis. Results: Information was collected for 100 medical records according to eligibility criteria. A mortality rate of 78% was evidenced in patients with post-intubated tracheal stenosis treated in the COVID ICU of the Carlos Lanfranco La Hoz Hospital in the period 2020-2021. Of these, the highest prevalence was in the female sex with 81.25%. Furthermore, a higher percentage of mortality was demonstrated in patients between 70 and 90 years of age, representing 93.55%. The main factors correlated with tracheal stenosis were difficult intubation and grade 4 according to Mallampati/Cormack with an OR of 56.83 and 88.00 respectively. Conclusions: In our region, tracheal stenosis has presented a high mortality rate (78%), which shows its severity in relation to the COVID-19 context. It is also a complication that significantly alters the quality of life of the survivors of this event. In addition, a higher preponderance of mortality was reflected in the female sex and the older adult age group. The factors related with tracheal stenosis with the highest significance were difficult intubation and grade 4 according to Mallampati/Cormack.
