Aloinmunización en pacientes politransfundidos con insuficiencia renal crónica hospital II-1 Moyobamba, 2020- 2023

Abstract

La insuficiencia renal crónica es una enfermedad degenerativa en aumento, relacionada con condiciones como diabetes e hipertensión. Los pacientes con esta condición suelen recibir múltiples transfusiones sanguíneas, lo que puede provocar aloinmunización, una respuesta inmunológica ante antígenos extraños. Esta situación incrementa el riesgo de reacciones hemolíticas y complica la seguridad transfusional. La aloinmunización es un desafío en medicina transfusional, ya que los anticuerpos contra antígenos eritrocitarios (Rh, Kell, Kidd, Duffy, entre otros) pueden generar reacciones adversas graves. Estudios en Latinoamérica y Ecuador han evidenciado una prevalencia de aloanticuerpos entre 2,98% y 4,3% en pacientes politransfundidos. Sin embargo, en el Hospital II-1 Moyobamba no existen estudios previos sobre este fenómeno en pacientes con insuficiencia renal crónica. Esta investigación busca determinar la prevalencia, especificidad y factores asociados a la aloinmunización en estos pacientes entre 2020 y 2023. Sus hallazgos podrían mejorar la seguridad transfusional, optimizar la selección de componentes sanguíneos y prevenir complicaciones post-transfusionales. Asimismo, contribuirá a la toma de decisiones en la institución y a la generación de conocimiento en el campo de la inmunohematología y hemoterapia.

Chronic kidney failure is a degenerative disease that is on the rise and is linked to conditions such as diabetes and hypertension. Patients with this condition often receive multiple blood transfusions, which can lead to alloimmunization, an immune response to foreign antigens. This situation increases the risk of hemolytic reactions and complicates transfusion safety. Alloimmunization is a challenge in transfusion medicine, since antibodies against red blood cell antigens (Rh, Kell, Kidd, Duffy, among others) can cause serious adverse reactions. Studies in Latin America and Ecuador have shown a prevalence of alloantibodies between 2.98% and 4.3% in patients with multiple transfusions. However, at Hospital II-1 Moyobamba, there are no previous studies on this phenomenon in patients with chronic kidney failure. This research seeks to determine the prevalence, specificity, and factors associated with alloimmunization in these patients between 2020 and 2023. Its findings could improve transfusion safety, optimize blood component selection, and prevent post-transfusion complications. It will also contribute to institutional decision-making and knowledge generation in the field of immunohematology and hemotherapy.
