Perfil de resistencia antimicrobiana de Raoultella spp. y características demográficas en aislamientos de pacientes atendidos en dos hospitales de Lima, 2019 – 2023

Abstract

Las infecciones intrahospitalarias representan una preocupación creciente, especialmente en países de ingresos bajos y medianos. Raoultella spp., bacteria filogenéticamente relacionada con Klebsiella spp., ha sido reportada con mayor frecuencia en entornos hospitalarios, afectando gradualmente a pacientes hospitalizados. Su similitud con Klebsiella spp. plantea la necesidad de investigaciones que permitan diferenciarla adecuadamente y entender su perfil de resistencia antimicrobiana. OBJETIVO: Determinar el perfil de resistencia a los antimicrobianos y las características demográficas de Raoultella spp. en aislamientos clínicos de dos hospitales de Lima Metropolitana, entre 2019 y 2023. MATERIAL Y MÉTODOS: Estudio observacional, descriptivo y transversal. Se analizaron 60 registros de pacientes con aislamiento de Raoultella planticola y Raoultella ornithinolytica, identificados por el sistema VITEK 2 Compact entre julio de 2019 y julio de 2023. RESULTADOS: 90% del total de muestras eran de la especie R. planticola (54/60). Predominaron los casos en adultos mayores y en pacientes hospitalizados. Muestras como secreción traqueal (28.3%) y sangre (23.3%) fueron las más frecuentes. La mayor resistencia se observó frente a trimetoprima/sulfametoxazol en R. planticola (90.3%) y a cefazolina en R. ornithinolytica (66.7%). CONCLUSIÓN: R. planticola predominó en pacientes hospitalizados y vulnerables, con muestras respiratorias y hemocultivos como principales fuentes. Aunque mostró amplia sensibilidad antimicrobiana, destacó la alta resistencia a trimetoprima/sulfametoxazol y cefazolina, sugiriendo patrones clínicamente relevantes y posibles mecanismos intrínsecos aún no confirmados.

Nosocomial infections remain a growing concern, particularly in low- and middle-income countries. Raoultella spp., a bacterium phylogenetically related to Klebsiella spp., has been increasingly reported in hospital environments, progressively affecting inpatients. The similarity of Raoultella spp. to Klebsiella spp. underscores the need for research that enables proper differentiation and a better understanding of its antimicrobial resistance profile. OBJECTIVE: To determine the antimicrobial resistance profile and demographic characteristics of Raoultella spp. in clinical isolates from two hospitals in Metropolitan Lima between 2019 and 2023.MATERIALS AND METHODS: Observational, descriptive, cross-sectional study. Sixty records of patients with isolates of R. planticola or R. ornithinolytica, identified by the VITEK 2 Compact system between July 2019 and July 2023, were analyzed. RESULTS: Ninety percent of the isolates corresponded to Raoultella planticola (54/60). Cases were more frequent among older adults and hospitalized patients. Tracheal secretions (28.3%) and blood (23.3%) were the most common sample types. The highest resistance was observed to trimethoprim-sulfamethoxazole in R. planticola (90.3%) and to cefazolin in R. ornithinolytica (66.7%). CONCLUSION: Raoultella planticola predominated in hospitalized and vulnerable patients, with respiratory secretions and blood cultures as the main sources. Although it showed broad antimicrobial susceptibility, the marked resistance to trimethoprim-sulfamethoxazole and cefazolin stands out, suggesting clinically relevant patterns and possible intrinsic mechanisms that remain to be confirmed.
