Objective: To determine the relationship between the urine protein-to-creatinine ratio (UPCR) in a single urine sample and 24-hour proteinuria in pregnant women with suspected preeclampsia treated at the Hospital Nacional Docente Madre Niño San Bartolomé during 2024. Additionally, to estimate its diagnostic performance for significant proteinuria and evaluate its agreement with the reference standard. Methodology: A quantitative, observational, retrospective, cross-sectional, and correlational study was conducted. A total of 404 records of pregnant women with available results for UPCR and 24-hour proteinuria were included through a census of eligible records. Due to the non-normal distribution of the variables, the relationship was assessed using Spearman’s rho test. The association between UPCR classification (≥0.30) and significant proteinuria (≥300 mg/24 h) was analyzed using the chi-square test. Discriminative capacity was evaluated using receiver operating characteristic (ROC) curve analysis, and diagnostic agreement was determined using Cohen’s kappa coefficient. Results: A moderate-to-high positive correlation was found between UPCR and 24-hour proteinuria (ρ=0.601; p<0.001). With an operational cutoff point of UPCR ≥0.30, a sensitivity of 79.3% and a specificity of 89.8% were obtained, with a positive predictive value of 74.6% and a negative predictive value of 92.0%, along with a statistically significant association (p<0.001). ROC analysis showed high discriminative capacity (AUC=0.908; 95% CI: 0.874–0.941) and an optimal cutoff point of 0.325. Diagnostic agreement was substantial (κ=0.677; 95% CI: 0.597–0.757; p<0.001), with an observed agreement of 0.869.Conclusion: UPCR was significantly associated with 24-hour proteinuria and demonstrated useful diagnostic performance, particularly for ruling out significant proteinuria, supporting its use as a practical tool for timely clinical evaluation in pregnant women with suspected preeclampsia.
